New Patient Registration
If you wish to register with this practice you will need to ask at reception for a registration pack. You can either fill this in whilst in the surgery or take it away and complete it at home.
You can make this quicker by printing and completing the registration form before you come to the surgery.

All patients are required to provide proof of eligibility for free NHS care to register as a regular patient. Documents acceptable listed below:
- Photo ID
- Tenancy Agreement for 6 months or more
- School or college letter confirming course of 6 months or more
- Benefits book/Pension book
- Work Permit
- Home Office Letter if you are seeking asylum or IND card
On registering with the practice all adults and children over the age of 5 will be given an appointment for a health check with the health care assistant or nurse. We ask that you keep this appointment and bring any relevant health documents with you, together with a urine sample in a small, clean container. If you are on 'repeat' medication please bring details of all your medication.
If you have children under the age of 6 please bring details of any immunisations they have had previously whether in this country or abroad. It is important the children are fully immunised to the schedule for this country.
All new patients are entitled to a basic health check on joining the practice, this is carried out by our Health Care Worker or one of our Practice Nurses. We encourage all new patients to take advantage of this service.
