New Patient Registration

If you wish to register with this practice during the coronovirus pandemic, we ask that you complete the registration form below

It is really helpful ( but not essential) for patients to provide proof of ID. Documents acceptable listed below:

  • Photo ID
  • Tenancy Agreement for 6 months or more
  • School or college letter confirming course of 6 months or more
  • Benefits book/Pension book
  • Work Permit
  • Home Office Letter if you are seeking asylum or IND card

Once completed please email the form (and any available ID) to . If you are unable to email, the form can be dropped at the surgery.

On registering with the practice all adults and children over the age of 5 will be given an appointment for a health check with the health care assistant or nurse. During the current pandemic this will be completed over the telephone. Please have relevant health documents with you, together with details of all your medication.

If you are registering children under the age of 6 please bring details of any immunisations they have had previously whether in this country or abroad. It is important the children are fully immunised to the schedule for this country.
